Job Description:
A company is looking for a Telephonic Care Navigator to provide
support and resources to members remotely.
Key Responsibilities:
Conduct telephonic outreach to members and assist with scheduling
provider appointments
Educate members on care gaps and connect them with community
resources
Maintain metrics for quality and performance while providing
excellent customer service
Required Qualifications:
High School Diploma / GED or equivalent work experience
At least 1 year of call center or telephonic customer service
experience
1 year of experience in a professional office setting using phone
and computer
1 year of experience in healthcare, insurance, social work, or
community outreach
Proficient in Microsoft Word, Excel, and Outlook
